Generating the report

Open Report, choose a date range and report contents, then tap Create report. Documented assembles a PIP- or focused SSDI-shaped Claim Evidence Report from the records you select.
Date range tips
A 28-day report shows a recent pattern. A longer range can show variability, recovery, and repeated consequences. Choose a period that fairly represents your experience, and include the full relevant period when preparing for an appeal.
Before sharing
Preview the PDF and check names, dates, selected mappings, photos, medications, and connected-app summaries. The exported file is readable and may contain sensitive information.
