Your data

Keep a copy of important records before changing devices or deleting data. Optional sync and sharing are separate from files you choose to export. The Documented privacy policy explains what is stored and the available deletion controls.

Will my evidence be accepted by an assessor or tribunal?

Documented helps you organise your own records and examples. An assessor or tribunal decides how to consider the evidence in your individual claim. A diary is not independent medical evidence, and the app cannot guarantee that a report will be accepted or that a claim will succeed. Check the current guidance for your claim and seek qualified support when needed.

Which benefits schemes does it support?

PIP and focused SSDI workflows are the supported mapped systems in version 1.0. PIP includes the daily living and mobility activities and descriptors. SSDI includes focused criteria and work-function prompts for selected evidence areas including epilepsy, lupus, connective tissue disease, chronic skin conditions, and ME/CFS.

Is this legal or medical advice?

No. Documented is an evidence-organising tool. It does not provide legal advice, medical advice, representation, diagnosis, a benefits decision, or any guarantee of claim outcome. Always check current official guidance and get qualified welfare-rights, benefits, legal, or clinical support when you need it.

What does Documented+ unlock?

Documented+ is a £9.99 lifetime unlock for formal PIP or focused SSDI criterion mapping, variability statistics, the Claim Evidence Report PDF, report attachments and medication summaries, optional compatible-app evidence, and PIP assessment, Mandatory Reconsideration, and tribunal tools. Daily evidence logging, photos, activity tagging, and CSV export stay free.

Is Documented a subscription?

No. Daily evidence capture stays free, and Documented+ is a single lifetime purchase. There is no Documented subscription and no recurring charge.

Is my data private?

Yes. Documented has no account, no analytics, no advertising SDK, and no Cocoacorn server for claim or health data. Records stay on your device unless you enable optional iCloud features or choose to export, save, print, back up, restore, purchase, or share. Purchases and billing

Documented includes a free tier. See the pricing page for optional upgrades. Purchases are made inside the app and billed by Apple.

If a purchase is missing, contact us so we can help. For refund requests, follow Apple’s refund instructions. Apple determines refund eligibility.
