Features

Document the right things, against the right criteria

PIP and SSDI claims are won and lost on whether your evidence maps to the assessment criteria. Documented gives you the structure that benefits advisors recommend but no app actually provides.

Criteria-mapped logging

PIP descriptors and focused SSDI evidence criteria for conditions including epilepsy, lupus, connective tissue disease, skin conditions, and ME/CFS

Photos and notes

Capture aids, medication, adapted equipment, visible symptoms, home impact, and what happened afterwards

Contemporaneous record

Daily entries timestamped on your device — courts and tribunals weight contemporaneous evidence heavily

Document the right things, against the right criteria

Build evidence over weeks, not nights before

Disability claims need consistency, not a panicked weekend of remembering. Documented helps you keep the record clean across the months a claim actually takes.

A continuous record

See how many weeks or months of consistent evidence you have built — without gamified streaks or pressure

Variability over time

Surface good-day/bad-day patterns that single snapshots miss

Assessment prep

Prompts and checklists for assessment, post-assessment notes, factual errors, MR, and appeals

Build evidence over weeks, not nights before

Export a claim-ready PDF when it matters

Generate a structured evidence file that mirrors the form your assessor is reading from. Take it to your GP, your solicitor, or attach it directly to your claim.

Assessment-aligned PDF

Sections matched to PIP descriptors or SSDI criteria, with your own daily evidence under each

Date-range exports

Export the last 3, 6, or 12 months as needed for application or appeal stages

Yours to share

No upload, no portal — the PDF is on your device for you to use however you choose

Frequently Asked Questions

Contact us
Will my evidence be accepted by an assessor or tribunal?
Contemporaneous symptom and limitation diaries are widely recognised by courts, tribunals, and benefits assessors as objective evidence — precisely because they were written down at the time, not reconstructed from memory after the fact. The strength of any individual claim depends on consistency, comprehensiveness, and the underlying medical case. Documented helps with the first two; we cannot guarantee an outcome.
Which benefits schemes does it support?
PIP and focused SSDI workflows are the first-class mapped systems. Claim profiles also support DSP and CPP-D, and the core diary, photos, profile, backup, and export workflow can still help with other benefits systems while deeper mapping evolves.
Is this legal or medical advice?
We’re not lawyers or doctors. Documented helps you keep an evidence record — what you logged, when, against which assessment criteria. It isn’t legal advice and isn’t a substitute for a benefits advisor, welfare rights officer, or solicitor. For complex claims, appeals, or tribunals, get qualified support — Citizens Advice, your local welfare rights team, or a disability law specialist.
Is my data private?
Yes. Documented has no accounts, no analytics, and no Cocoacorn servers. Data stays on your device unless you choose iCloud sync in your personal Apple account. See the full privacy statement at cocoacorn.com/privacy.
