When you turn on 'Show evidence prompts' for a person in More > People, the observation sheet adds a small set of PIP/DLA/SEND-aligned chips — preparing food, washing and bathing, managing toilet needs, dressing and undressing, communicating, reading, mixing with others, making decisions, moving around, planning and following a journey.
Tap any chip while logging an observation to record what they needed help with or couldn't do. You can also attach one photo from your library if you have useful visual evidence, with an optional caption.
Over time the chips and photos build up into evidence you can hand to a benefits adviser, school, or care professional — far more grounded than trying to remember six months back when you're filling the form.
Only enable this if it's useful for your situation; it's off by default.
