1. Start with the day
Choose an overall difficulty only if it feels useful, then write one or two sentences about what was difficult, what help you needed, or what happened afterwards.
2. Add an affected activity
Choose the PIP activity or focused SSDI evidence area that best matches the experience. Daily activity tagging is available without Documented+.
3. Save and continue later
You can return to the log, add another activity, or attach a photo. There is no requirement to complete every field.
Record a daily evidence log
Record a dated example in your normal journal. Add only details that describe your experience.
An open journal with separate coloured spaces beside a kitchen timer represents recording an activity, help needed and what happened afterwards.
Record a daily evidence log: steps
- Check the date Choose the day you are describing and the overall difficulty that fits your experience.
- Describe the effect Write what happened, help you needed and the consequence. Activities and photos are optional. Use your own experience, including what you could do.
- Save your log Use Save log in the toolbar. The guide finishes after saving succeeds. A journal entry does not submit evidence or predict a claim decision.
