Documented helps you keep dated records of how a health condition or disability affects everyday activities.
What it does
You can log daily experiences, attach photos, organise entries against supported PIP activities or focused SSDI criteria, and export your records. Documented+ can turn them into a structured Claim Evidence Report.
What it does not do
Documented does not provide legal or medical advice, represent you, or decide whether you qualify for a benefit. The relevant authority makes that decision using all the available evidence.
