Tended is a journal for recording how your condition affects you day to day. The more consistently you log, the more useful your insights become.

What to log

Rate your day from 1–5, log any symptoms you’re experiencing and their severity, and optionally add triggers, food, medications, relief methods, mood labels, energy level, sleep quality, bowel type, activities, reactions, period days, and a reflection. You don’t need to log everything every day — even a day rating on its own is valuable.

Condition starter sets

When you add conditions, Tended can suggest starter symptoms, triggers, foods, activities, relief methods, and custom fields. PMOS / PCOS appears with both names because PMOS is the updated name for PCOS, while many clinicians, records, and searches still use PCOS during the transition.

How often

Most people use one journal entry per day and update it as the day changes. That is the default Tended is designed around. Multiple entries per day are supported when you want separate morning/evening notes or distinct events, but you do not need to split a normal day into several entries. Tended will never guilt you for missing a day.

In-progress entries

If you save a note, medication, symptom, or reflection before choosing a day rating, Tended keeps it visible on your timeline as an in-progress entry. You can return later, add the rating, and finish the day without creating a second entry.

What Tended does with your data

After a few weeks of entries, Tended starts spotting patterns: what differs between your lower-rated and higher-rated days, which factors often appear together, and how symptoms and measures change over time. These observations appear on the Insights hub, where each card takes you deeper into a specific area.

Quick logging

On tough days, you don’t need to open the app. Rate your day from the home screen widget, check pending medications from the medication widget, or use Siri: “Rate my day on Tended.” Apple Watch Ultra users can assign the Action Button to log a flare with a single press. Switch to day-by-day view for a calmer, one-day-at-a-time experience — swipe between days. Long-press the + button on the timeline to use “Same as yesterday” — it creates a new entry pre-filled with yesterday’s symptoms, triggers, food, medications, activities, and more.

What Tended is not

Tended is a personal health journal, not a medical device. It doesn’t provide medical advice, diagnosis, or treatment. It helps you review observations from your own records and share them with a healthcare professional.
